History:

Previous ovarian cyst removed 2 years ago on the left. Now similar pains in the right iliac fossa. Previously diagnosed PCOS.

Answer:

It seems like most opted for the obvious answer which is a dermoid cyst as most classic ultrasound features of a dermoid lesion are present here i.e. the mass is hyperechoic, well defined and delineated from the ovarian tissue however the correct answer is infact a small lipoma. One clue may be the fact that no obvious distal acoustic shadow is present which is present in the case of most dermoid cyst lesions - however from an operator perspective it is difficult to distinguish either and one would perhaps comment on the possibility of either in the report.
